- Case Management - These teams, made up of trained health care specialists, help you and your patients follow a diabetes management care plan, maximize plan benefits, and coordinate goal-setting and follow-ups.
Services and Tools That Extend Your Care Plan
In addition to health management programs, we offer health care services and tools to help your patients stick to the care or treatment plan you designed.
Options include:
- Home-based Health Care - Depending on your patient's condition or needs, they may be eligible for home-based health care, such as home delivery pharmacy, in-home wellness exams and screenings, and durable medical equipment.
