These health plans offer you cost-efficient access to a local network that's limited to quality doctors, specialists and hospitals, right where you live, and who understand the needs of your community.

Medical Network Plan
- In-network primary care provider (PCP) required for yourself and your covered dependents.
- Referrals needed from your PCP for specialists, except for OB/GYN services.
- Out-of-network services will only be covered in emergencies*.
- Prior authorizations (pre-approval) may be needed for hospitalizations and some types of outpatient care.
- Plans include global emergency and urgent care coverage*, 24/7.
Medical Network Point of Service Plan
- A primary care provider (PCP) is required for yourself and your covered dependents.
- Referrals needed from your PCP for specialists, except for OB/GYN services.
- Out-of-network services are covered but you will have higher out-of-pocket costs and you may need to file a claim.
- Prior authorizations (pre-approval) may be needed for hospitalizations and some types of outpatient care.
- Plans include global emergency and urgent care coverage*, 24/7.
